We have to create a probation review format whereby all the employees who have completed their probation will be reviewed on parameters apart from individual performance. Can anyone help me with the format for the same?

We are thinking of issuing the confirmation letter only once the HR and line managers finish the probation review and map a training competency map for each employee before releasing their confirmation letters. Can I have your advice on the same?

The immediate supervisor can evaluate the individual on the following parameters:
- Quality of Work
- Teamwork
- Discipline; adherence to norms
- Communication, presentation skills
- Values - honesty & integrity
- Initiative
- Technical Aspects
- Growth Potential

Highlight achievements and provide recommendations for improvement. Also, obtain feedback from the client if the individual is directly working with the client. Obtain feedback from the Project Manager/Department Head as well. Obtain feedback from the HR Manager.

Based on the performance, ratings, and feedback, confirm the services of the employee OR extend probation.

Nikita
