First of all, get the correct bank details linked to your UAN and get it verified with the employer. Then, visit the EPFO. They will generate the UDMO pre-filled form. Just attach a cancelled cheque leaf and get the UDMO issued by EPFO signed by you and attested by the employer. Submit the same to EPFO. As EPFO is now almost fully computerized, they don't accept any manually filled forms or requests.
