Dear Rangusomu,

While issuing the warning letter, it is important to cite acts of indiscipline. We should specify what occurred, where and when it happened, in whose presence it occurred, whether there was a violation of any rules, regulations, SOP, etc., and whether the employee was absent. Issuing a warning letter for an "indiscipline attitude" is vague. It is better to avoid vagueness in the warning letter.

Taking a cue from the above, I recommend that you write the warning letter yourself and upload it here. Some seniors, like me, will review and provide feedback.
