You could probably address a polite letter to HR saying something like this:

I joined the department on [date] and my probation period was for [number] months, which got completed on [date]. I request you to confirm that my probationary period has been completed, and I have been confirmed as of [date].

My advice is to learn to write correct English. Your habit of capitalizing your words is poor English. Use full stops to end sentences. Do not start sentences with "As." Writing good English following grammatical rules will help a lot in your career.
