ESIC NILL RETURN FORM 5 - cannot find the correct Form 5 format for the same purpose

Hello Friends, Recently, the esic office (Pune) asked me to submit a nil return in Form 5 format. Has anyone done this before? I cannot find the correct Form 5 format for the same purpose. Please help me. Thanks & Regards.
