I want to change my father's name in my PF account, as my previous employer registered me with the wrong father's name. Now, I want to withdraw my PF, but due to the wrong father's name, it is not possible. EPFO has asked me to prepare and submit an indemnity bond in this regard. Can anyone help me prepare a draft of the indemnity bond to change my father's name on my PF account?

Dear, Please find enclose here with Sworn Affidavit which you may submit to P.F. office. You shall have to execute it on non judicial stamp paper as rule prevailing in your State. A. Prakash
