Form 5 - 10 is for newly joined and left employees in the last month, and Form 13 is 'not to withdraw P.F. from the past/last establishment.' When an employee submits Form 13 to the newly joined establishment, the HR Department of the new company has to put his name in Form 5 as a newly joined employee with Form 13 as the last employed PF to transfer to the new establishment. It's called Form 5-10 because it is front-back printed on a single page.

Please take Form 5 and Form 10 (Search Option) to break up the details of Non-Contribution days during the EPF service period from your previous Employer. Enclose this information with the Transfer Form-13 R to process it with the present Employer.
