Please edit your questionnaire to avoid typos; for example, see Q25 "25) Do any of the following hinder you in balancing your work and family commitments?"
Asking personal and demographic questions at the outset may put some respondents off. Hence, it is better to move them to the end.
You have not stated why you are conducting the survey and what you will do with the findings. Only polar (Yes/No) questions are not suitable for degree-level projects. Please read some material available at CiteHR by clicking on the links in the sidebar to the right.
Finally, kindly use a web-based survey if you want to improve the response rates. Expecting people to download your questionnaire, answer, save, and send it back to you is not ideal.
